VA FORMS


The following are a list of forms most commonly used by the VA.  Each button is linked to the “fillable” PDF form. If you pause your cursor over each button it will explain what each form is used for.

The purpose for VVW placing them here is NOT so you can fill it out and file your own claim. VVW does not recommend representing or filing your own claims with the VA. The forms are here so you can fill in the information, print it out and take it to your local Veterans Service Organization (VSO) office for representation. This is to help expedite the process of filing.

When obtaining representation, it is not necessary to go to the local VA Regional office to be placed with a VSO. You can go to the VSO of your choice. Some of the top one’s are: VFW, American Legion, DAV, MOPH, VVA, and PVA etc… Also don’t forget to bring any supporting documentation with you when you go to file.
